Position Overview: This position is responsible for providing services and support to homeless individuals and families. This includes Coordinated Entry Lead duties including maintaining the regional by-name-list. Case management tasks are focused on strengths and barriers to housing stability.   Services rendered are designed to foster independent living and improve the quality of life for these individuals, respecting each individual’s preferences in level of independence and location, consistent with an evidenced-based Housing First model. The ideal candidate is detail oriented, a self-starter, and happy to work in a team atmosphere. Persons with lived experience, BIPOC, and bi-lingual are encouraged to apply.

Key Responsibilities:
  • Manage grant funds to utilize a minimum of 100% of the grant for the term of the contract.
  • Complete eligibility and entry assessments for by-name-list inclusion. 
  • Maintain the regional by-name-list for homeless services related to NCCEH, HUD, and DHHS funding.
  • Identify with Coordinated Entry subcommittee the next appropriate household for available housing slot.
  • Provide assistance with move-in process by linking to appropriate resources.
  • Serve as liaison and consult with community agencies. 
  • Assist households in obtaining safe, decent, and affordable housing that follows their preferences in level of independence and location, consistent with an evidenced-based Housing First model.
  • Assist households in developing amicable relationships with local landlords.
  • Assist new tenants in negotiating and understanding the terms of the lease and paying rent and utilities.
  • Provide transportation resources as needed in relation to obtaining and maintaining housing stability.
  • Attend and actively participate in CE related meetings and/or trainings as required.
  • Be the primary point of contact with NCCEH staff in regards to the by-name-list, SSO-CE grant, and other pertinent funders.
  • Manage additional tasks as needed.
  • Actively participate in team meetings, training sessions, and ongoing professional development opportunities.
Qualifications:
  • Combination of Bachelor’s degree in Human Services or experience working in human services or lived experience
  • Knowledge of local housing resources, tenant rights, and fair housing practices
  • Excellent interpersonal and communication skills
  • Technology and computer skills
  • Ability to work independently and collaboratively within a team
  • Cultural competence and sensitivity to the needs of diverse populations
  • Must have a valid driver’s license with a 5-year clear driving record
  • Must be able to lift 50 pounds
Pay is $21/hour
