Position Overview: This position is responsible for providing case management through services and supports to individuals and families referred through the Healthy Opportunities Pilot. Case management tasks are focused on strengths and barriers to housing stability. Services rendered are designed to foster independent living and improve the quality of life for these individuals, respecting each individual’s preferences in level of independence and location, consistent with an evidenced-based Housing First model.

Key Responsibilities:
- Manage grant funds to utilize a minimum of 90% of the grant for the term of the contract.
- Manage grant funds to minimize spending over 100% of the grant for the term of the contract.
- Submit all reimbursement requests and supporting documents in a timely manner to be determined by the Housing Director.
- Identify with supervisor and Healthy Opportunities Pilot process through NCCARES360 the next appropriate household for available housing slot.
- Provide assistance with move-in process.
- Provide case management for all tenants, including coordinating and monitoring of supportive services and maintaining contact as appropriate to their particular status and needs.
- Serve as liaison and consult with community agencies pertinent to potential and current tenants and supportive services.
- Maintain a list of landlords that are willing and able to work within HOP guidelines.
- Assist tenants in obtaining safe, decent, and affordable housing that follows their preferences in level of independence and location, consistent with an evidenced-based Housing First model.
- Assist tenants in developing amicable relationships with local landlords.
- Assist new tenants in negotiating and understanding the terms of the lease and paying rent and utilities.
- Transport tenants as needed in relation to obtaining and maintaining housing stability.
- Provide support and advocacy for tenants’ rights.
- Perform inspections as authorized through NCCARES 360.
- Work toward strengthening each tenant’s sustainability in housing.
- Assist tenants to achieve goals as outlined in their case management.
- Attend scheduled case management meetings and other meetings and training as required.
- Manage additional tasks as needed.

Qualifications:
- Combination of Bachelor’s degree in Human Services or experience working in human services or lived experience
- Knowledge of local housing resources, tenant rights, and fair housing practices
- Knowledge of needs of homeless individuals with behavioral health/substance use challenges
- Excellent interpersonal and communication skills
- Technology and computer skills
- Ability to work independently and collaboratively within a team
- Cultural competence and sensitivity to the needs of diverse populations
- Must have a valid driver’s license with a 5-year clear driving record
- Must be able to lift 50 pounds
